New Patient Paperwork – NP Form 2024
During the initial visit starts with your consultation, Dr. Powers will perform a detailed examination and then specific neurological-based testing. This helps us determine the best course of action for your treatment. Throughout your treatment plan, we will perform follow-up exams so we can map your progress and improvements.
We often take X-rays for diagnostic purposes. However, if you’ve had X-rays recently, we might be able to use your previous images instead of taking new ones. If you have any questions or would like help transferring your X-rays to our office, give us a call! We are proud to offer in-house X-ray for the convenience of our patients.
Following the initial visit will be the Doctor’s report of findings; including a detailed interpretation of the testing results and the doctor’s recommendations on treatment and frequency of care.
